Summary description of the IPAC lapse |
- Reusable instruments (e.g., air/water syringes) were not sterile to the point of use.
- Reusable instruments (e.g., air/water syringes) were stored in a drawer, which was observed to be visibly dirty.
- Incorrect packaging of the reusable air/water syringes prior to sterilization (i.e., the instruments were not properly packaged to allow steam to move around and through the item(s) and all contact surfaces).
- Manufacturer’s instruction for use (MIFU’s) for the reusable air/water syringes were not available for review.

Following the investigation conducted by health unit staff, the following corrective actions were taken:
- Reusable air/water syringes will no longer be used
- Single use disposable air/water syringes were acquired for use by dental clinic staff
