​​Summary description of the IPAC lapse
  1. ​Reusable instruments (e.g., air/water syringes) were not sterile to the point of use.
  2. Reusable instruments (e.g., air/water syringes) were stored in a drawer, which was observed to be visibly dirty.
  3. Incorrect packaging of the reusable air/water syringes prior to sterilization (i.e., the instruments were not properly packaged to allow steam to move around and through the item(s) and all contact surfaces).
  4. Manufacturer’s instruction for use (MIFU’s) for the reusable air/water syringes were not available for review.
​​Additional comments and/or updates to initial report ​Staff members responsible for any or all steps in reprocessing are recommended to complete the Medical Device Reprocessing Techniques Online Course through the Medical Device Reprocessing Association of Ontario.
Brief description of corrective measures taken ​Following the investigation conducted by health unit staff, the following corrective actions were taken:

  • Reusable air/water syringes will no longer be used
  • Single use disposable air/water syringes were acquired for use by dental clinic staff
